Two forms of personal identification are required, such as a valid driver’s license, state ID, Social Security card, passport, or permanent resident visa/green card
Complete copies of your Forms 1040 from the last two years, including all pages
Current Asset Statements*
Checking, Savings, or CD Accounts
Stocks, Bonds, or Money Market Accounts
IRA or 401(k) Accounts
*Please provide documentation for the source of any large deposits appearing on asset or bank statements.
Income & Employment
Complete employment history for the past 2 years
Pay stubs for the most recent 30 days
W-2s, 1099s, or K-1s from the last 2 years
Federal tax returns with all schedules for the past 2 years
Contact information for your employer’s Human Resources department
Most recent award letters or decrees, if applicable (Social Security, pension/retirement, disability benefits, or divorce decree)
Self-Employment
Business federal tax returns for the last 2 years (include all pages of Forms 1120, 1120S, or 1065, depending on your business structure if self-employed)
Year-to-date profit and loss statement
Comprehensive list of all business debts
Currently Owned Property
Most recent mortgage statements for all liens on all properties owned (include all pages)
Latest homeowner’s insurance declarations page for each property owned (if applicable)
Current HOA dues statement
Under Contract
Fully executed sales contract
Homeowner’s insurance quote for the new home
Earnest money deposit: If included in the contract, provide a copy of the canceled check and a transaction history showing the funds have been withdrawn (if not visible on the bank statement provided)
Credit Information
Letter of explanation for any late payments, judgments, collections, or other derogatory credit items
Judicial decrees or court orders for any obligations resulting from legal actions
Bankruptcy and discharge papers for all past bankruptcies in your credit history
Gift Letters
Acceptable gift sources include relatives, non-profit organizations, and government entities
Gift letters must be completed and signed by both the giver and recipient
Applicable Documents
For VA: member 4 copy of DD-214, all pages (for former military) or Statement of Service (for active military)
Child support order for the last 6 months, including monthly amount and children’s ages
Alimony payment details: monthly amount and duration remaining
